Under-18 Liabilty Waiver

Players of this age must have a liability waiver form completed by either a parent or guardian to become an eligible, registered player within the Utah Soccer Association.

The completed form must include the proper signatures. The original, completed form must be delivered to the Association. The address of the office is found on the form. A copy of the form can be obtained by clicking on the following link:
