Photo Release Form

I, the undersigned, hereby grant permission to the Utah Soccer Association to use photographs of me or my likeness for use in the Utah Soccer Association's web site and other various media publications and displays without further consideration or compensation, and I acknowledge the Association's right to crop or treat the photograph at its discretion.

I agree on the behalf of the minor registering to the Utah Soccer Association to release and hold harmless the Utah Soccer Association and its agents or employees from any claims arising from the related use of the photographs. I give the Utah Soccer Association full copyright and authority to publish photography and I agree to the terms listed above

I hereby acknowledge that I have read the information above and accept the bylaws, rules, policies, procedures, etc. as described within this waiver. I KNOWINGLY ASSUME THE RISKS AND AM SIGNING THIS ACKNOWLEDGEMENT VOLUNTARILY.
